Reports

Seirrah OT can provide reports for multiple purposes; including, gaining a second opinion, for personal use, for schools, to demonstrate intervention and progress, following the administering of standardised assessments and to support the educational needs of a child.

Assessment Reports

As part of our Occupational Therapy assessments we will provide a report outlining the findings and recommendations. This report is brief and is needed to begin a block of therapy sessions.

Upon request, we are able to produce an in depth report from the assessment which can be used for multiple purposes.

Special Educational Needs Statement Reports

A Special Educational Needs (SEN) statement provides details of the educational needs of a child and any adaptive requirements needed to ensure they are adequately and correctly supported whilst accessing education.

Occupational Therapy input is sometimes needed for children with a Statement of SEN. Seirrah OT are able to produce a report to assist with identifying the Occupational Therapy needs of individuals and the necessary provision to meet those needs.

Education, Health and Care Plan Reports

An Education, Health and Care Plan (EHCP) provides details of the health and educational needs of a child and any adaptive requirements needed to ensure they are adequately and correctly supported whilst accessing education.

Occupational Therapy input is sometimes needed for children with an EHCP. Seirrah OT are able to produce a report to assist with identifying the Occupational Therapy needs of individuals and the necessary provision to meet those needs.

Special Educational Needs and Disability Tribunal (SENDIST) Reports

At Seirrah OT we work with several of the UK’s leading educational law firms and parent advocacy services. Our specialist Occupational Therapists are able provide assessments, reports, and stand as expert witness during Special Educational Needs and Disability Tribunals (SENDIST).

Our experts ensure that any case they are working on meets the required deadlines for evidence being submitted and provide an independent, accurate, detailed report outlining adequate quantified provision to meet the individual needs of the child in order to access education.

Multi-professional Reports

Seirrah OT are an independent private Occupational Therapy service who collaborate with other professional disciplines to produce assessments, treatment, training, and reports for children and young people. As part of our work with other therapy companies we are able to provide a holistic approach to evaluating client’s needs and agreeing a single set of goals.

Our Occupational Therapists are able to input directly onto other professionals reports to ensure the report is concise and identifies all of the child’s needs and recommendations within one document.
